چکیده

مقدمه: کاربرد فاکتورهای رشدی در زمینه درمان‌های رژنراتیو مورد بررسی قرار گرفته‌اند و این عوامل بر تکثیر، کموتاکسی، تمایز و سنتز ماتریکس خارج سلولی و سلول‌های مؤثر در التیام زخم نقش داشته و در نتیجه باعث تسهیل ترمیم انساج پریودنشیوم و رژنراسیون آن‌ها می‌شوند. هدف مطالعه حاضر، مروری بر مطالعات موجود در مورد تأثیر کاربرد پلاسمای غنی از پلاکت در کنار مواد پیوندی متفاوت در حضور و عدم حضور غشاء در درمان‌های رژنراتیو ضایعات داخل استخوانی بود. شرح مقاله: جستجوی الکترونیکی در PubMed و Medline برای مطالعات کارآزمایی بالینی کنترل شده با حداقل دوره پیگیری 6 ماهه که از پلاکت تغلیظ شده در درمان ضایعات داخل استخوانی استفاده کرده بودند، صورت گرفت. مقالات تا پایان ماه آوریل سال 2011 جمع‌آوری شدند. معیارهای ورود در مطالعات مورد بررسی شامل حداقل عمق ضایعه استخوانی 3 میلی‌متر و حداقل از دست رفتن چسبندگی 5 میلی‌متر بود. تعداد 20 مقاله واجد شرایط شناخته شده مورد تأیید و بررسی قرار گرفتند. پارامترهای مورد بررسی شامل تغییرات عمق پروب، پر شدن ضایعه استخوانی و افزایش حد چسبندگی بود. نتیجه‌گیری: در محدوده مطالعات انجام شده در این رابطه، کاربرد پلاسمای غنی از پلاکت اثر مضاعفی را در بهبود پارامترهای کلینیکی در درمان‌های رژنراتیو در ضایعات داخل استخوانی به هنگام کاربرد غشا نداشته است، اگرچه در عدم کاربرد غشاء، کاربرد پلاسمای غنی از پلاکت در همراهی با مواد پیوند استخوانی اثرات مثبتی را به دنبال داشته است. کلید واژه‌ها: بیماری پریودنتال، پلاسمای غنی از پلاکت، کارآزمایی بالینی

A review of efficacy of platelet-rich plasma in the treatment of intrabony periodontal defects

Introduction: Use of growth factors has been evaluated in regenerative treatments and it has been shown that they are effective in proliferation, chemotaxis, differentiation, and synthesis of extracellular matrix and cells responsible for wound healing, enhancing the repair and regeneration of the periodontium. The aim of this literature review is to evaluate the effects of platelet-rich plasma (PRP) in combination with different graft materials, with and without membranes, in regenerative therapy of intrabony defects. Review of Report: Electronic search was carried out in PUBMED and MEDLINE for randomized clinical trials which have used platelet-rich plasma in the treatment of intrabony defects with a minimum follow-up period of 6 months. Articles were collected until the end of April 2011. Inclusion criteria in the evaluated research studies consisted of defects at least 3 mm in depth and pockets at least 5 mm in depth. Twenty articles were confirmed and evaluated in this review. The parameters evaluated were probing depth changes, defect fill and an increase in clinical attachment level. Conclusion: Within the limitations of studies carried out in this respect it can be concluded that use of PRP has not exert a positive adjunctive effect on improving clinical parameters in regenerative treatments of intrabony defects when used in combination with membranes; however, when it has not been used with a membrane, use of PRP along with bone grafts has led to positive results.   Key words: Clinical trial, Periodontal disease, Platelet-rich plasma
