گزارش اولیه یک مورد مولتیپل میلوما در فکین به صورت تومورهای مرکزی و محیطی

چکیده
چکیده مقدمه: مولتیپل میلوما بدخیمی سلول‌های پلاسمای خون است که در 30-15 درصد موارد آن درگیری فک گزارش شده است. در این مقاله گزارش یک مورد از این بیماری و مراحل تشخیصی و درمانی آن بیان شده است. گزارش مورد: بیمار مرد هفتاد ساله‌ای بود که با شکایت از تومور بافت نرم فک پایین که باعث ایجاد اشکال در جویدن و درد می‌شد، مراجعه نمود. در معاینه، ندول‌های متعدد در دهان و جمجمه داشت. در بررسی رادیوگرافی ضایعات متعدد رادیولوسنت در استخوان فک و جمجمه مشاهده شد. بررسی خون و ادرار انجام گرفت؛ پس از نمونه‌برداری، مولتیپل میلوما برای وی تشخیص داده شد. بررسی‌های بیشتر و درمان دارویی توسط متخصص سرطان‌شناسی انجام شد. نتیجه‌‌گیری: هر چند احتمال مراجعه اولیه بیمار با علایم دهانی مولتیپل میلوما به مطب دندان‌پزشکی نادر می‌باشد، ولی شناخت علایم دهانی و سیستمیک این بیماری باعث تشخیص و درمان سریع آن شده، طول عمر و کیفیت زندگی بیمار را بهبود می‌بخشد.کلید واژه‌ها: مولتیپل میلوما، فک بالا و پایین، تومور مرکزی، تومور محیطی

Central and peripheral tumors of multiple myeloma in the Jaws (A case report)

Abstract Introduction: Multiple myeloma is a malignancy of plasma cells which affect the jaws in 15-30% of the cases. This paper explains a case of the multiple myeloma and its diagnosis. Case Report: The patient was 70 years old; he complained from painful nodule in the mandible interfered in chewing. In clinical examination some nodules were seen in the skull and jaws. In radiographic examination multiple destructive lesions were seen in the skull and jaws. The differential blood count (CBC diff), the urine analyzes, and biopsy was done. The diagnosis was multiple myeloma. More examinations and treatment process were done by oncologist. Discussion: Although this is rare that the first signs of multiple myeloma are in the mouth and a patient comes to the dentist for diagnosis, but knowledge of the signs of this malignancy leads to fast diagnosis and treatment and improving the life time and the quality of life.
