Abstract
Introduction: pain control in dental treatment especially in root canal treatment is an important issue. The aimed of study to investigate the effect of dexamethasone infiltration along with the inferior alveolar nerve block IANB (lidocaine with epinephrine 1/80000) on the post treatment pain after root canal therapy was done on posterior teeth with irreversible pulpitis.
Materials and Methods: in this clinical trial study, 60 patients with irreversible pulpitis and acute pain in inferior molar teeth were selected and randomly divided into two groups. After standard block injection for each patient, buccal infiltration injections of dexamethasone or saline was performed. Preoperative pain by visual analogue scale (VAS) was recorded from 0 to 10 before starting treatment, and at 4, 6, 12 and 24 hours after treatment. Data were analyzed with independent t-test, Chi-square tests, Mann-Whitney, Friedman, Regression tests (α= 0.05).
Result: The two groups were matched for age, sex, tooth and pain before treatment and there was no significant difference. Pain in the control group 12 and 24 hours after treatment was significantly less than pain four hours, however in the dexamethasone group this difference was not significant. By controlling confounding factors, preoperative pain significantly increases the pain after treatment (at 4 and 6 hours) and gender impact on pain in 12 hours after treatment. By controlling other factor, the amount of pain control by dexamethasone injection, in 4 and 6 hours after treatment was 1/211 and 1/154 units higher than the control group.
Conclusion: Buccal infiltration of dexamethasone, on pain control 4 and 6 hours after treatment was significant. Preoperative pain significantly affect the pain at 4 and 6 hours after treatment in mandibular molars with irreversible pulpitis.
