بررسی خصوصیات کلینیکوپاتولوژیک کارسینوم سلول سنگفرشی دهان، در گروه‌های سنی مختلف در یک دوره‌ی 10 ساله

چکیده

مقدمه: بیش از 95 درصد از سرطان‌های دهان، کارسینوم سلول سنگفرشی (Squamous cell carcinoma) SCC می‌باشد. شیوع SCC دهان در بالغین جوان کمتر از 40 سال، بین 0/4 تا 3/9 درصد از کل بیماران مبتلا به این سرطان می‌باشد. در سال‌های اخیر، افزایش شیوع این ضایعه‌ی بدخیم در سنین جوانی دیده شده است. هدف از این مطالعه، ارزیابی شاخص‌های کلینیکوپاتولوژیک SCC دهان در گروه‌های سنی مختلف در بیماران مراجعه کننده به دانشکده‌ی دندان‌پزشکی اصفهان بوده است.
مواد و روش‌ها: در این مطالعه توصیفی- تحلیلی، تعداد 80 پرونده‌ی ثبت شده با تشخیص نهایی SCC مورد بررسی قرار گرفت. اطلاعات دموگرافیک و بالینی شامل سن، جنسیت، نمای بالینی، مکان ضایعه و طول مدت آن، از پرونده‌های موجود در بخش پاتولوژی دهان دانشکده‌ی دندان‌پزشکی اصفهان استخراج شد. شاخص‌های هیستوپاتولوژیک و درجه‌ی بدخیمی تومور توسط دو پاتولوژیست دهان مورد بررسی قرار گرفت. اطلاعات به دست آمده، وارد نرم‌افزار SPSS نسخه‌ی 22 شد و با آزمون‌های آماری کای‌اسکوئر، من‌ویتنی و فیشر مورد تجزیه و تحلیل آماری قرار گرفت. میزان 0/05 < p value، معنی‌دار تلقی شد.
یافته‌ها: از تعداد 80 نمونه، 42 (52/5 درصد) زن و 38 (47/5 درصد) مرد با نسبت زن به مرد = 1/1 بود. مخاط آلوئولار، شایع‌ترین مکان درگیر در همگی گروه‌های سنی بود. شایع‌ترین تظاهر بالینی به صورت ضایعه‌ی اگزوفیتیک و در تمام گروه‌های سنی، کارسینوم سلول سنگفرشی خوب تمایزیافته، شایع‌ترین نوع هیستوپاتولوژیک بود. هیچ یک از شاخص‌های کلینیکوپاتولوژیک بین گروه‌های سنی، تفاوت معنی‌دار نداشت (p value < 0/05).
نتیجه‌گیری: طبق نتایج مطالعه‌ی اخیر، هیچ پارامتر هیستوپاتولوژیک اختصاصی بین گروه‌های سنی مختلف در بیماران مبتلا به SCC وجود نداشت. باوجود افزایش شیوع SCC دهان در بیماران جوان، این تومور بدخیم همچنان یکی از بیماری‌های مختص سنین بزرگسالی می‌باشد.

Evaluation of Clinico-Pathological Characteristics of Oral Squamous Carcinoma in Different Age Groups in a 10-Year Period

Introduction: Over 95% of oral cancers are squamous cell carcinomas (SCC). The prevalence of oral SCC in subjects < 40 years of age has been reported to be 0.4‒3.9% of all the subjects with this lesion. The number of young patients with this malignant tumor has increased in recent years. The present study was undertaken to evaluate the clinico-pathological parameters of OSCC in patients in different age groups, referring to Isfahan Faculty of Dentistry.
Materials and Methods: In this retrospective, cross-sectional study, we reviewed demographic data, clinical features and histopathological parameters of 80 OSCC cases in different age groups from the Archives of the Oral Pathology Department, Isfahan Faculty of Dentistry. The histopathological parameters and malignant tumor grades were evaluated by two oral pathologists. Data were analyzed with SPSS 22, using chi-squared and Mann-Whitney tests (α = 0.05).
Results: Of all the cases evaluated, 42 (52.5%) were female and 38 (47.5%) were male, with a female-to-male ratio of 1.1. Alveolar mucosa with 31 (38.8%) cases was the most common site of involvement in all the age groups. The most prevalent clinical feature of OSCC in the patients was exophytic lesion. Well-differentiated (grade 1) OSCC was the most common subtype of OSCC in all the age groups. There was no significant difference in the clinico-pathological parameters between the age groups (p value > 0.05).
Conclusion: Based on the results of the present study, there was no specific histopathological parameter between different age groups with OSCC. Despite the increase in the number of young patients with OSCC, it is still considered a specific malignant tumor in old age groups.
