بررسی تغییرات رفتاری مرتبط با ترس و اضطراب در کودکان تحت بی‌هوشی عمومی در دندان‌پزشکی

چکیده

مقدمه: ترس و اضطراب کودکان از درمان‌های دندان‌پزشکی، می‌تواند باعث تغییرات رفتاری شود. همچنین درد و ناراحتی‌های دندانی نیز باعث اختلالات رفتاری در کودکان می‌شود. ممکن است انجام درمان‌های دندانی تحت بی‌هوشی عمومی، به سبب بهبود در شرایط پزشکی کودک، اختلالات رفتاری او را کاهش دهد. هدف از این مطالعه، تعیین و مقایسه‌ی میانگین نمره‌ی رفتارهای ترس و اضطراب بر اساس پرسش‌نامه‌ی استاندارد (Behavioral change) قبل و بعد از درمان دندان‌پزشکی تحت بی‌هوشی عمومی بود.
مواد و روش‌ها: این مطالعه‌ی توصیفی-تحلیلی از نوع مقطعی، شامل کودکانی بود که برای انجام درمان‌های دندان‌پزشکی، تحت بی‌هوشی عمومی قرار گرفته بودند. تعداد 78 پرسش‌نامه‌ی استاندارد شامل 22 گویه، بین مادران توزیع شد. این پرسش‌نامه یک‌بار درست قبل از بی‌هوشی و یک‌بار یک ماه بعد از بی‌هوشی توسط آنها تکمیل شد، تا از این طریق میانگین نمره‌ی تغییرات رفتاری به دست آید. داده‌های به دست آمده به واسطه‌ی نرم‌افزار SPSS نسخه‌ی 23 و آزمون‌های آماری تی، تی زوجی و ANOVA، تجزیه و تحلیل شدند (0/05 = α).
یافته‌ها: با توجه به داده‌های آماری، نمره‌های اختلالات رفتاری بعد از بی‌هوشی عمومی، در تمامی موارد نسبت به قبل از بی‌هوشی، شرایط بهتری داشت و همچنین با توجه به (r = 0/249 ،p value = 0/028) بین تعداد کار دندان‌پزشکی و میانگین نمره‌ی تغییرات رفتاری، ارتباط معنی‌دار وجود داشت به این معنا که با افزایش تعداد درمان‌های دندان‌پزشکی، بهبود شرایط رفتاری بیشتر می‌شد.
نتیجه‌گیری: انجام کامل درمان‌های دندان‌پزشکی کودک تحت بی‌هوشی عمومی، باعث بهبود در شرایط رفتاری او می‌شود، که اصلی‌ترین دلایل آن می‌تواند بهبود شرایط دندانی بیمار باشد.

Introduction: Fear and anxiety of dental treatment in children could cause behavioral changes. In addition, dental pain and discomfort could lead to behavioral disorders in children. Treatments under general anesthesia might decrease behavioral disorders due to an improvement in the child’s medical condition. The aim of this study was to determine and compare the mean scores of fear and anxiety based on the standard behavioral change questionnaire before and after dental treatment under general anesthesia.
Materials & Methods: This descriptive cross-sectional study evaluated children who underwent dental treatment under general anesthesia. Seventy-eight behavioral change questionnaires, consisting of 22 items, were distributed among the children’s mothers. The questionnaires were completed once just before general anesthesia and once a month after general anesthesia by the mothers in order to obtain the mean score of behavioral changes. Data were analyzed with t-test, one-way ANOVA and paired t-test (α = 0.05).
Results: Based on statistical data, in all the cases the scores of behavioral problems after general anesthesia were better than those before general anesthesia. In addition, there was a significant relation between the number of the dental procedures and the mean score of behavioral changes, indicating an improvement in behavior with an increase in the number of dental procedures.
Conclusion: Implementation of all the dental procedures for children under general anesthesia improves their behavioral conditions, presumably due to an improvement in their dental condition.
