فراوانی و خصوصیات بالینی ضایعات داخل فکی در یک دوره‌ی 7 ساله

چکیده

مقدمه: استخوان فکین می‌تواند در معرض انواع بیماری‌های ژنتیکی، متابولیک، التهابی تومورال و کیستیک قرار گیرد. آگاهی از فراوانی، خصوصیات بالینی، دموگرافیک این ضایعات می‌تواند در تشخیص صحیح این ضایعات و درمان مناسب آن‌ها مفید باشد. مطالعه‌ی حاضر با هدف تعیین خصوصیات بالینی و دموگرافیک ضایعات فکین در یک دوره‌ی هفت ساله در اصفهان انجام شد.
مواد و روش‌ها: این مطالعه‌ی توصیفی- تحلیلی گذشته‌نگر بر روی 370 پرونده‌ی موجود در آرشیو بخش پاتولوژی دانشکده‌ی دندان‌پزشکی دانشگاه آزاد اسلامی واحد اصفهان طی سال‌های 97-1390 انجام شد. اطلاعات دموگرافیک، بالینی و تشخیص هیستوپاتولوژی ضایعات از پرونده استخراج و در چک‌لیست ثبت گردید. سپس داده‌ها به صورت توصیفی بیان و با آزمون آماری ANOVA و  Chi- Squareتجزیه و تحلیل شد (0/05 = α).
 یافته‌ها: فراوانی نسبی ضایعات داخل استخوانی 28/6 درصد  (106 مورد) 41/5 درصد در زنان و 58/4 درصد در مردان به دست آمد (0/05 > p value). میانگین سنی 35/96 در آقایان و 33/70 در خانم‌ها بود (0/05 < p value). تورم به عنوان تنها نشانه‌ی بالینی گزارش شده ( در ۱۵ درصد موارد) بود. ضایعات بیشتر در فک پایین (0/05 > p value) و در سمت راست دیده شد. از 106 پرونده مورد بررسی به طور کلی 73 مورد کیست ادونتوژنیک، 19 مورد، ضایعات التهابی و فیبرواسئوس، 9 مورد، تومور ادونتوژنیک، 2 مورد تومور خوش‌خیم غیر ادونتوژنیک و 3 مورد کیست غیرادونتوژنیک بود. کیست پری‌اپیکال و کیست دانتی‌ژور، شایع‌ترین بودند.
 بحث و نتیجه‌گیری: نتایج مطالعه‌ی حاضر نشان داد، ضایعات داخل استخوانی، بیشتر در آقایان و در سمت راست فک پایین دیده شد .تفاوت توزیع این ضایعات با مطالعات مشابه بیانگر تفاوت جغرافیایی در بروز ضایعات بود که نیاز به تحقیق و بررسی بیشتر دارد.

چکیده [English]

Introduction: Jawbones can be affected by many lesions such as genetic, metabolic, inflammatory, cysts, and tumors. For proper diagnosis and treatment, t is necessary for dentists to be aware of the most prevalent lesions, demographic and clinical features, and their distribution patterns in every population. The present study aimed to evaluate the distribution pattern of jaw bone lesions in 7 years in Isfahan, Iran.
Materials and Methods: This retrospective analytic descriptive study was performed on 370 reports available from the department of oral pathology at Isfahan Azad university. demographic data, clinical features, and histopathology diagnosis were gathered and analyzed statistically by ANOVA and Chi-square (α = 0.05).
Results: Intraosseous lesions consisted 28.6% of all lesions.41.5% in females and 58.4% in males (p < 0.05. (Mean age of male patients was 35.96 and 33.70 for females (p value > 0.05). Lesions were mostly in the right lower jaw (p value < 0.05). The swelling was the only clinical feature reported in only 15% of 106 intraosseous lesions, 73 cases were odontogenic cysts,19 inflammatory,9 odontogenic tumors,2 benign non-odontogenic tumors, and 3 non-odontogenic cysts. The periapical and dentigerous cyst was the most prevalent lesions reported.
Conclusion: Results of the present study showed jaw bone lesions mostly occurred in the right lower jaw in male patients. it is understood that the distribution of intraosseous lesions of the oral cavity is related to genetic, environmental, and geographic factors. Further studies are required.
