بررسی میزان شیوع افسردگی در بیماران مبتلا به لیکن پلان دهانی در بیماران مراجعه کننده به کلینیک دندانپزشکی دانشگاه اصفهان در سال 98-97

چکیده

مقدمه: لیکن پلان دهانی، یک بیماری خودایمنی و التهابی مزمن نسبتاً شایع است که تغییر پاسخ ایمنی با واسطه‌ی سلولی نقش اصلی را در پاتوژنز آن ایفا می‌کند. در میان عوامل احتمالی مؤثر در ایجاد این بیماری، نقش اختلالات روانی به خصوص افسردگی و اضطراب در پاتوژنز بیماری مورد بحث قرار گرفته است. بنابراین پژوهش حاضر با هدف بررسی میزان شیوع افسردگی در بیماران مبتلا به لیکن پلان دهانی در بیماران مراجعه‌کننده به کلینیک دندان‌پزشکی دانشگاه اصفهان در سال 98-1397 بود.
مواد و روش‌ها: در پژوهش مورد- شاهدی حاضر، تعداد 40 بیمار مبتلا به لیکن پلان دهانی مراجعه کننده به کلینیک ویژه‌ی دندان‌پزشکی دانشگاه علوم پزشکی اصفهان در سال 1397-98 (گروه مورد) و تعداد 40 فرد غیرمبتلا به بیماری لیکن پلان دهانی و کاملاً سالم (گروه شاهد) مورد بررسی قرار گرفت. سپس پرسش‌نامه افسردگی بک (BDI) توسط افراد مورد مطالعه تکمیل گردید. در نهایت داده‌های بدست آمده با استفاده از نرم‌افزارهای آماری تجزیه و تحلیل شد. سطح معنی‌داری در مطالعه‌ی حاضر 0/05 در نظر گرفته شد.
یافته‌ها: نتایج بدست آمده نشان داد که میانگین سنی گروه مورد (53/43 سال) کمتر از گروه شاهد (44/68 سال) می‌باشد. بعلاوه میانگین افسردگی در گروه مورد (12/78) بیشتر از گروه شاهد (9/5) بود، با این حال این تفاوت از لحاظ آماری معنی‌دار نبود (0/05 < p value).
نتیجه‌گیری: بطور کلی این مطالعه نشان‌دهنده‌ی نقش افسردگی در پاتوژنز بیماری لیکن پلان دهانی بود. بطوری که سطح بالای افسردگی می‌تواند نقطه شروع واکنش‌های خودایمنی مختلف و ابتلا به لیکن پلان دهانی باشد.

چکیده [English]

Introduction: Oral lichen planus is a relatively common chronic autoimmune and inflammatory disease in which altered cellular mediated immune response plays a major role in its pathogenesis. Among the possible factors influencing development of oral lichen planus, the role of mental disorders, especially depression and anxiety, has been discussed in pathogenesis of this disease. Therefore, the aim of this study was to investigate the prevalence of depression in patients with oral lichen planus referred to the dental clinic of Isfahan University in year 2018-2019.
Methods: In the present case-control study, 40 patients with oral lichen planus referred to the dental clinic of Isfahan University in 2018-2019 (case group) and 40 subjects without oral lichen planus (control group) were investigated. Next, the Beck Depression Inventory (BDI) was completed by all subjects. Finally, the obtained data were analyzed using statistical software.
Findings: The obtained results indicated that the average age in case group (43.53 years) was lower than the control group (44.68 years). In addition, the mean for depression in case group (12.78) was higher than control group (9.5), however, this difference was not statistically significant (p value > 0.05).
Conclusion: In general, this study indicated the role of depression in the pathogenesis of oral lichen planus. It was noted that high levels of depression can be an initiation point for various autoimmune reactions and oral lichen planus.
