بررسی مقایسه‌ای آنتی‌ژن p53 در ادنتوژنیک کراتوسیست و آملوبلاستوما به روش ایمونوهیستوشیمی

چکیده

مقدمه: ادنتوژنیک کراتوسیست، نوعی کیست تکاملی ادنتوژنیک با رفتار کلینیکی تهاجمی و احتمال عود بیشتر از سایر کیست‌های ادنتوژنیک است. هدف از این پژوهش، توجیه رفتار بیولوژیک ادنتوژنیک کراتوسیست با مقایسه‌ی بیان ژن پروتئین p53 به عنوان القاگر آپوپتوز به روش ایمونوهیستوشیمیایی در آملوبلاستوما و ادنتوژنیک کراتوسیست بود.
مواد و روش‌ها: در این مطالعه‌ی تجربی- آزمایشگاهی، 14 ادنتوژنیک کراتوسیست و 15 آملوبلاستوما از بلوک‌های پارافینه‌ی بخش پاتولوژی دانشکده‌ی دندان‌پزشکی قزوین در سال 94-1393 استخراج و پس از تأیید، تحت رنگ‌آمیزی ایمنوهیستوشیمی قرار گرفتند و سپس از لحاظ شدت و نحوه‌ی رنگ‌آمیزی بررسی شدند. در نهایت داده‌ها با استفاده آزمون‌های آماری Kruskal–Wallis و Mann–Whitney تجزیه و تحلیل شد (0/05 < p value).
 یافته‌ها: در میان نمونه‌های آملوبلاستوما، در 86/7 درصد بیان نشاگر p53 مشاهده گردید (77 درصد بیان ضعیف و 33 درصد بیان متوسط- 84/ 6 درصد به صورت کانونی 15/4 درصد به صورت منتشر). در میان نمونه‌های ادنتوژنیک کراتوسیست در 4/71 درصد بیان نشانگر p53 مشاهده شد (80 درصد ضعیف، 20 درصد متوسط- 50 درصد کانونی و 50 درصد منتشر) بودند. بین ادنتوژنیک کراتوسیست و آملوبلاستوما از نظر شدت و نحوه‌ی انتشار نشانگر p53 اختلاف معنی‌داری مشاهده نگردید (0/05 < p value).
نتیجه‌گیری: بیان نشانگر p53 در تومور آملوبلاستوما و ادنتوژنیک کراتوسیست تقریباً یکسان بوده و احتمالاً رفتار تهاجمی ادنتوژنیک کراتوسیست به خاطر افزایش بیان ژن p53 جهش یافته در سلول‌های اپی‌تلیالی ادنتوژنیک می‌باشد و ممکن است تا حدی رفتار بیولوژیک ادنتوژنیک کراتوسیست را توجیه نماید.

چکیده [English]

Introduction: Odontogenic keratocyst (OKC), which is a developmental cyst has aggressive clinical behavior and its recurrence possibility is higher than other odontogenic cysts. The aim of this study was estimate biologic behavior of OKC with p53 immunohistochemistry comparison, as a tumor suppressor between OKC and ameloblastoma.
Materials and Methods: In this experimental study, paraffin blocks of 14 OKC and 15 ameloblastoma, were collected from oral pathology department Qazvin University. After confirmation of H&E slides, they were stained immunohistochemically. Then the IHC slides were observed in terms of intensity and staining pattern. Finally, data were analyzed with Wilcoxon and Man-Whitney test (p value < 0.05).
Results: In 86.7 % of ameloblastoma with positive expression p53 marker, 77% were weak expression and 33% were moderate expression. Among positive staining samples 84.6% were focally and 15.4% were scattered. In the case of OKC 11.4% indicated p53 marker that 80% showed weak expression and 20% showed moderate expression. 50% were focally and 50% were scattered. These findings support was not significially different between intensity and pattern of p53 marker between ameloblastoma and OKC (p value > 0.05).
Conclusion: Expression of p53 was similar in ameloblastoma and OKC. Probably, high expression of p53 in odontogenic epithelial cells was one of the reasons for the OKC aggressive behavior. This findings vindicate biologic behavior of OKC.
