Abstract
Introduction: A dentigerous cyst is the most common developmental odontogenic cyst. Dentigerous cysts are usually asymptomatic but may cause pain and swelling if infected. Radiographically, the dentigerous cyst is usually a unilocular radiolucency with a well-defined sclerotic border associated with the crown of an unerupted tooth, although inflammatory cysts will show ill-defined borders.
Case Report: A 35-year-old man referred to have his wisdom teeth extracted. In the panoramic radiograph, there was a lesion in the distal of the third molar on the left mandible. The swelling was not evident in intraoral examination, but there was a gap near the lesion from 4 years ago, from which the patient stated that pus came out periodically. No pain or paresthesia was reported. In CBCT, radiolucency was observed with scattered internal fine calcifications with relatively clear boundaries. The patient underwent incisional biopsy and marsupialization and the sample was referred to a pathologist for histopathology. In the microscopic examination, the histopathological features of the inflamed dentigerous cyst were observed.
Discussion: Due to the changes in the internal structure and inflammatory conditions, dentigerous cysts can have variable radiographic views. Its definitive diagnosis is based on histopathological examination. Early detection and conservative surgery can lead to favorable results in the treatment of patients.
