مرور نظام‌مند و متاآنالیز بر ارتباط بین سطح سرمی ویتامین D و شیوع S-ECC در کودکان کمتر از 6 سال

چکیده

مقدمه: این مرور سیستماتیک و متاآنالیز با هدف بررسی ارتباط بین سطح سرمی ویتامین D و شیوع پوسیدگی زودرس شدید (Sever Early Childhood Caries) S-ECC در کودکان زیر 6 سال انجام شد.
مواد و روش‌ها: ییک جستجوی سیستماتیک در چندین پایگاه داده برای شناسایی مطالعات مرتبط انجام شد. معیارهای ورود شامل: مطالعات مورد-شاهدی منتشر شده بین سال‌های 2012 و 2024، گزارش پوسیدگی به صورت وجود و یا عدم وجود S-ECC در کودکان کمتر از 6 سال و همینطور میزان دقیق سطح سرمی ویتامین D بدست آمده از طریق اندازه‌گیری‌های آزمایشگاهی است. در نهایت دو نویسنده مطالعات را انتخاب کرده، داده‌ها را استخراج و خطر سوگیری را به طور مستقل ارزیابی کردند. سپس کیفیت مطالعات نیز بر اساس چک‌لیست STROBE ارزیابی شد و در نهایت مطالعات منتخب در متاآنالیز قرار گرفتند.
یافته‌ها: از بین 235 مطالعه شناسایی شده، 10 مطالعه در نهایت در متاآنالیز قرار گرفتند، نتایج متاآنالیز، ارتباط معنی‌داری را بین سطوح پایین‌تر ویتامین D سرمی و شیوع بالاتر S-ECC در کودکان زیر شش سال نشان دادند (میانگین اختلاف: nmol/lit73/12) (CL:[-17.10,-8.35]) که نشان‌دهنده وجود رابطه بین سطوح پایین ویتامین D و بروز S-ECC در کودکان است.
نتیجه‌گیری: این بررسی سیستماتیک و متاآنالیز ارتباط معنی‌داری را بین سطوح پایین ویتامین D سرمی و شیوع بالاتر S-ECC در کودکان کمتر از 6 سال نشان داد. با این حال، ماهیت مقطعی مطالعات وارد شده و وجود تنوع در روش اجرای مطالعات، نیاز به تحقیقات بیشتر برای اثبات علیت به طور قطعی را برجسته می‌کند. این یافته‌ها بر اهمیت در نظر گرفتن سطوح ویتامین D سرمی در بهبود سلامت دندان کودکان تأکید می‌کند و از طرح‌های مطالعاتی قوی‌تر در تحقیقات آینده حمایت می‌کند.

Association between Serum Vitamin D levels and the Prevalence of Severe Early Childhood Caries in Children under Six Years Old: A Systematic Review and Meta-analysis

Introduction: This systematic review and meta-analysis aimed to investigate the association between serum vitamin D levels and the prevalence of severe early childhood caries(S-ECC) in children under six years old.
Materials and Methods: A systematic search was conducted across multiple databases to identify relevant studies. Inclusion criteria encompassed case-control studies published between 2012 and 2023; caries outcomes expressed as prevalence of S-ECC among children under 6; and vitamin D levels assessed by laboratory analysis. Two authors selected studies, extracted data and assessed risk of bias independently. The quality of the studies was also assessed according to STROBE checklist. Selected studies were included in the meta-analysis.
Results: Among the 235 identified studies, 10 studies were finally included in the meta-analysis, Meta-analysis results revealed a significant association between lower serum vitamin D levels and higher prevalence of S-ECC in children under six years old (mean difference: 12.73 nmol/L, CI: [-17.10, -8.35]).which indicates the existence of a relationship between low levels of vitamin D and the occurrence of S-ECC in children.
Conclusion: This systematic review and meta-analysis suggest a significant association between lower serum vitamin D levels and higher prevalence of S-ECC in young children. However, the cross-sectional nature of included studies and variations in methodologies highlight the need for further research to establish causality definitively. These findings underscore the importance of considering serum vitamin D levels in pediatric dental health strategies and advocate for more robust study designs in future research.
