بررسی اثربخشی تحریک لرزش، سرما و بی‌حسی سطحی بر میزان درد بی‌حسی قدام ماگزیلا: کارآزمایی بالینی تصادفی

چکیده

مقدمه: تزریق بی‌حسی در دندان‌پزشکی همراه با درد، ناراحتی و اضطراب است. تاکنون مداخلات زیادی در جهت کاهش این ناراحتی صورت گرفته است. هدف از این مطالعه، بررسی میزان اثربخشی سه روش تحریک لرزش، سرما و استفاده از بی حسی سطحی بر درد ناشی از تزریق در ناحیه قدامی فک بالا بود.
مواد و روش‌ها: این مطالعه‌، کارآزمایی بالینی تصادفی یکسوکور است که در تابستان 1400 در بخش اندودانتیکس دانشکده‌ی دندان‌پزشکی دانشگاه علوم پزشکی اصفهان، روی 200 بیمار نیازمند درمان ریشه یک دندان قدامی فک بالا انجام شد. قبل از تزریق در ناحیه وستیبول باکال، در گروه اول سرما، در گروه دوم لرزش، در گروه سوم ژل بی‌حسی سطحی قرار داده شد و در گروه چهارم (کنترل) تزریق بدون مداخله صورت گرفت. درد ناشی از ورود سوزن و درد تزریق ماده‌ی بی‌حسی در ناحیه‌ی قدامی ماگزیلا توسط (Visual Analog Scale) VAS ثبت شد. آنالیز داده‌ها توسط آنالیز واریانس و آزمون تعقیبی Tukey انجام شد (0/05 = α).
یافته‌ها: در ارتباط با درد ورود سوزن ژل بی‌حسی سطحی و لرزش به یک اندازه در کاهش درد ورود سوزن در قدام ماگزیلا مؤثر بوده‌اند و در مقایسه با گروه سرما و شاهد مؤثرتر هستند (0/05 > p value).
 در درد حین تزریق بی‌حسی، ژل بی‌حسی سطحی کمترین میانگین نمره درد و سپس لرزش و سرما و گروه شاهد به ترتیب کمترین نمره‌ی میانگین درد را داشتند (0/05 > p value). بین نمره‌ی درد ورود سوزن و تزریق ماده‌ی بی‌حسی در قدام ماگزیلا با جنسیت بیماران هیچ ارتباط معنی‌داری یافت نشد (0/05 < p value).
نتیجه‌گیری: کاربرد ژل بی‌حسی سطحی و دستگاه لرزش در کاهش درد ورود سوزن و درد حین تزریق بی‌حسی موضعی در تزریق انفیلتراسیون قدام ماگزیال مؤثر بودند.

Evaluation the Vibratory Stimulation, Cooling and Topical Anesthesia Efficacy on Pain Due to Injection in Anterior Maxilla: A Randomized Clinical Trial

Introduction: Injections in dentistry are accompanied by pain, discomfort, and anxiety. Many interventions have been undertaken so far to reduce this discomfort. This study aimed to assess the effect of vibration, cold, and topical anesthesia on the reduction of pain experienced during infiltration injections in the anterior maxilla.
Materials and Methods: This study is a randomized clinical trial conducted in the summer of 2023 in the Endodontic department of the Faculty of Dentistry, Isfahan University of Medical Sciences, on 200 patients who needed root canal treatment of an upper maxillary tooth. Before the injection in the buccal vestibule area, in the first group cold, in the second group vibration, in the third group topical, anesthetic gel was placed, and in the fourth group (control) only the injection was done without intervention. The pain level experienced when the needle enters and during the injection of the anesthetic in the anterior region of the maxilla was recorded using the Visual Analog Scale (VAS). The data was analyzed using variance analysis and the Tukey tracking test (α = 0.05).
Results: Concerning needle insertion pain, superficial anesthesia gel and vibration have been equally effective in reducing pain in the front of the maxilla and are more effective compared to the cold and control groups (p value < 0.05). With pain during anesthesia injection, surface anesthetic gel had the lowest average pain score, followed by vibration and cold, and the control group had the lowest average pain score, respectively (p value < 0.05). No significant relationship was found between the pain score of needle insertion and anesthetic injection in the front of the maxilla and the gender of the patients (p value < 0.05).
Conclusion: The use of topical anesthesia and vibration devices is effective in reducing the pain of needle insertion and during injection in the infiltration of the anterior maxilla.
