Introduction: Hypercementosis is an adaptive or pathological change in the tooth root characterized by excessive cementum deposition. This condition is usually asymptomatic and is often detected incidentally on radiographs. However, its presence can be challenging in endodontic treatments. The aim of this report is to present a case of diffuse Hypercementosis in the right mandibular first molar and to describe the successful root canal treatment in such a condition.
Case Presentation: The patient was a 55-year-old man who referred to the specialized endodontic department of Zahedan Dental School with complaints of severe pain in the area of the right mandibular first molar. Clinical examination and pulp vital tests confirmed the diagnosis of irreversible pulpitis. Periapical radiographs showed diffuse Hypercementosis in both mesial and distal roots, while other teeth lacked such changes. Endodontic treatment was performed using the Crown-down technique using 2.5% sodium hypochlorite, and the canals were obturated with gutta-percha and AH26 sealer using the lateral compression method. At the six-month follow-up, the tooth was free of any clinical signs or periapical changes.
