Introduction: Hypercementosis is an adaptive or pathological change in the tooth root characterized by excessive cementum deposition. This condition is usually asymptomatic and is often detected incidentally on radiographs. However, its presence can be challenging in endodontic treatments. The aim of this report is to present a case of diffuse hypercementosis in the right mandibular first molar and to describe the successful root canal treatment in such a condition.
