گزارش مورد درمان ریشه دندان پرمولر دوم ماگزیلا مبتلا به دایلسریشن و انحنای شدید ریشه

چکیده
مقدمه: موفقیت درمان ریشه تا حد زیادی به شناخت دقیق آناتومی کانال ریشه و مدیریت صحیح پیچیدگی‌های آن وابسته است. دایلسریشن ریشه یکی از ناهنجاری‌های رشدی نادر است که می‌تواند با ایجاد انحنای شدید، درمان اندودانتیک را با چالش‌های جدی و افزایش خطر خطاهای حین درمان مواجه سازد. دندان پرمولر دوم ماگزیلا به‌طور معمول دارای آناتومی ساده‌تری است و بروز دایلسریشن در آن نسبتاً نادر گزارش شده است .
شرح مورد: در این گزارش، درمان اندودانتیک موفقیت‌آمیز یک دندان پرمولر دوم فک بالا در بیمار خانم ۳۲ ساله با درد شدید هنگام جویدن ارائه می‌شود. بررسی‌های بالینی و رادیوگرافیک، نکروز پالپ، پریودنتیت آپیکال حاد و وجود ریشه با انحنای شدید S شکل ناشی از دایلسریشن را نشان داد. درمان با ایجاد دسترسی مستقیم، تعیین طول کارکرد با اپکس‌لوکیتور، پاکسازی و شکل‌دهی کانال با استفاده از فایل‌های دستی پیش‌خم‌شده و سیستم روتاری نیکل–تیتانیوم حرارت‌دیده و شستشوی فعال کانال انجام شد. پرکردگی کانال به روش تراکم جانبی و با سیلر بایوسرامیک صورت گرفت.

Introduction: The success of root canal treatment largely depends on a thorough understanding of root canal anatomy and the proper management of its complexities. Root dilaceration is a relatively rare developmental anomaly that can significantly complicate endodontic treatment by creating severe root curvature and increasing the risk of procedural errors. Maxillary second premolars usually present with relatively simple anatomy, and the occurrence of dilaceration in these teeth is considered uncommon.
Case Presentation: This report describes the successful endodontic management of a maxillary second premolar in a 32-year-old female patient who presented with severe pain during mastication. Clinical and radiographic examinations revealed pulp necrosis, acute apical periodontitis, and a severely S-shaped curved root associated with dilaceration. Treatment was performed using straight-line access preparation, working length determination with an electronic apex locator, and canal cleaning and shaping with pre-curved hand files and heat-treated nickel–titanium rotary instruments, accompanied by activated irrigation. The canal was obturated using the lateral compaction technique with a bioceramic sealer.
