Introduction: The success of root canal treatment largely depends on a thorough understanding of root canal anatomy and the proper management of its complexities. Root dilaceration is a relatively rare developmental anomaly that can significantly complicate endodontic treatment by creating severe root curvature and increasing the risk of procedural errors. Maxillary second premolars usually present with relatively simple anatomy, and the occurrence of dilaceration in these teeth is considered uncommon.
Case Presentation: This report describes the successful endodontic management of a maxillary second premolar in a 32-year-old female patient who presented with severe pain during mastication. Clinical and radiographic examinations revealed pulp necrosis, acute apical periodontitis, and a severely S-shaped curved root associated with dilaceration. Treatment was performed using straight-line access preparation, working length determination with an electronic apex locator, and canal cleaning and shaping with pre-curved hand files and heat-treated nickel–titanium rotary instruments, accompanied by activated irrigation. The canal was obturated using the lateral compaction technique with a bioceramic sealer.
